Create your own member history by completing and submitting the following electronic form.  Work with your Public Relations department or agency to make the most of this opportunity.  Upon completion, you will receive a confirmation email.  The CHA History Committee reviews submissions quarterly.  After the review, you will be notified of acceptance, acceptance with changes, or you will be encouraged to re-submit.
